LIABILITY DISCLAIMER AND MEDICAL AUTHORIZATION:
I do hereby relieve Queen of All Saints Parish, the QAS Activities Association board and it's members from any and all liabilities, including personal injury, arising out of or pertaining to participation of your child in athletic programs and activities of Queen of All Saints in connection to this registration.

I further authorize the manager or coach of my child's team to secure any and all necessary medical aid, assistance, or treatment for my child during any practice or game if such medical aid, assistance, or treatment is required due to any injury to my child. I have read the above foregoing Liability Disclaimer and Medical Authorization and understand the contents thereof, and agree that the information and facts contained on my registration are true to the best of my knowledge and belief.
 

CYC CARDS:
Every player participating in CYC sports must have a CYC identification card.  You will need a birth certificate, baptismal certificate, driver's license, or an old CYC card.  The cost of a card is $3.00.  CYC cards are not required for intramural sports.  Managers and coaches are required to have a Coaching Concept card.  Please reference Coaching Requirements for more information.
Sign-up dates and times will be posted on both on qasaa.org  and at cycstl.net.

WORK DUTY REQUIREMENT:
My family obligation funds our sports programs and helps to provide our children with the best possible facilities, equipment and uniforms. It is our family's obligation to fulfill our Stewardship obligation by working in the concession stand or on specified field days.  Failure to comply will result in our child not being allowed to participate in subsequent sports, and / or forfeiture of equivalent funds ($150) either through deposit checks or as a reference charge against my credit card.  Please reference the Work Duty Policy for more information.
 

UNIFORM RETURN:
Uniforms are the property of the QAS Activities Association and must be returned at the end of the sports season.  Uniform turn-in dates will be scheduled by my child's coach.  It is my responsibility to ensure that my child's uniforms are returned in clean undamaged condition.  Failure to turn in a uniform will result in my child not being allowed to participate in subsequent sports, and / or forfeiture of equivalent funds ($75) either through deposit checks or as a reference charge against my credit card.

PAYMENT AGREEMENT:
As parents and/or guardians of children registered in sports activities at QAS, I hereby agree to support the QAS Activities Association program by participating in concession stand duty or performing comparable tasks,  as determined by the Athletic Association board.

I understand and agree that failure to complete assigned concession stand duty will result in my child not being allowed to participate in subsequent sports, and / or forfeiture of equivalent funds ($150) either through deposit checks or as a reference charge against my credit card.

I also understand and agree that failure to return an undamaged uniform  will result in my child not being allowed to participate in subsequent sports, and / or forfeiture of equivalent funds ($75) either through deposit checks or as a reference charge against my credit card.
